WM4 Online Application Form


Welcome to the online application form for the Four Thousand Footer Club of the White Mountains.

To use this form, fill in your information, hike dates and comments/notes. Once you are satisfied that the form is complete, you MUST click the Download PDF button to download the application file.

After some validation of your data, your web browser will download a custom PDF file of your WM4 application. During the validation process it will be possible to cancel and make changes to your application data. It is also possible to save your application to a PDF even if it is incomplete, and finish it later by scanning the QR code from the PDF.

For convenience your application data will remain in the form, until you click the Clear Application button.

Print this application from the downloaded PDF file (both pages) and mail it along with your essay and application fee to AMC Four Thousand Footer Committee P.O. Box 444 Exeter, NH 03833-0444.
